- Participation: Participation is limited to parents with an active Modak account.
- Age Requirement: Participants must be at least 18 years old.
3. Purpose of the Survey
The purpose of this survey is to gather feedback from customers to understand their experience, which will help us enhance our product and overall user experience.
4. Data Collection and Use
- Collection of Information: We collect certain personal information from you when you participate in the survey to facilitate the survey process and analyze the results. This may include your name, email, and phone number.
- Use of Information: The information collected from you will be used to analyze and improve user experience and our product.
- Data Retention: We will retain your survey responses for as long as necessary to fulfill the purposes outlined in this section, unless a longer retention period is required or permitted by law.

- Confidentiality: All responses will be kept confidential. Individual responses will not be shared publicly.
- Anonymity: Wherever possible, surveys will be conducted anonymously. If anonymity is not feasible, this will be clearly communicated to you before you participate in the survey.

8. Raffle Participation
- Raffle Entry: By completing the survey, customers will be automatically entered into a raffle to win 500 MBX, which will be credited to the teen account and can be redeemed for USD using the MBX to USD feature within the teen app.
- Winner Selection: The winner will be selected at random from all eligible survey participants. It will be done through Google Sheets.
- Announcement of Winner: The winner will be announced on June 7th. After that date, responses will not be considered for the raffle.
9. Claiming the Prize
- Notification: The winner will be notified via email as provided in the survey responses.
- Prize Credit: The 500 MBX will be credited to the teen account within 72 hours of the winner's announcement.
- Redeeming MBX: The credited MBX can be redeemed for USD using the MBX to USD feature within the teen app.
10. Payment Period
Payment Timing: The prize will be credited to the winner's account within 72 hours after the announcement of the winner.
11. Technical Issues
Modak is not responsible for any technical issues that may affect participation in the survey, including but not limited to, internet outages, server downtime, or software malfunctions. We will make every effort to address and resolve technical issues promptly.
12. Compliance with Laws
Participation in the survey is subject to all applicable laws and regulations. Participants are responsible for ensuring compliance with any local, state, or federal laws that may apply.
13. Amendment or Cancellation
Modak reserves the right to amend or cancel the survey at its discretion, without prior notice. In the event of cancellation, Modak will make reasonable efforts to notify participants.
14. Consent to Terms
By participating in the survey, participants agree to abide by these terms and conditions. Modak reserves the right to take appropriate action against any participant who violates these terms.
