These terms and conditions govern your participation in our internal survey campaign designed to improve our services and understand customer satisfaction and opinions. By accessing and using our services, including participating in this survey, you agree to abide by these terms and conditions. Please read them carefully. If you do not agree with any part of these terms, we kindly ask that you refrain from participating in the survey.

2. Eligibility

• Participation: Participation is limited to parents with an active Modak account.
• Age Requirement: Participants must be at least 18 years old.

3. Purpose of the Survey

The purpose of this survey is to gather feedback from customers to understand their experience, which will help us enhance our product and overall user experience.

4. Data Collection and Use

• Collection of Information: We collect certain personal information from you when you participate in the survey to facilitate the survey process and analyze the results. This may include your name, email, and phone number.
• Use of Information: The information collected from you will be used to analyze and improve user experience and our product. 
• Data Retention: We will retain your survey responses for as long as necessary to fulfill the purposes outlined in this section, unless a longer retention period is required or permitted by law.

• Confidentiality: All responses will be kept confidential. Individual responses will not be shared publicly.
• Anonymity: Wherever possible, surveys will be conducted anonymously. If anonymity is not feasible, this will be clearly communicated to you before you participate in the survey.

8. Claiming the Prize

• Eligibility for reward: By completing the survey, customers will be automatically earn $5 added to their Modak Parent Account associated with email provided in the survey.
• Prize Credit: $5 added to Modak Parent Account, for the parent to send to their kid’s MoCard at their discretion.
  
  

9. Payment Period

Payment Timing: The $5 will be added within 2 business days after survey closure on June 25th, 2024.

10. Technical Issues

Modak is not responsible for any technical issues that may affect participation in the survey, including but not limited to, internet outages, server downtime, or software malfunctions. We will make every effort to address and resolve technical issues promptly.

11. Compliance with Laws

Participation in the survey is subject to all applicable laws and regulations. Participants are responsible for ensuring compliance with any local, state, or federal laws that may apply.

12. Amendment or Cancellation

Modak reserves the right to amend or cancel the survey at its discretion, without prior notice. In the event of cancellation, Modak will make reasonable efforts to notify participants.

13. Consent to Terms

By participating in the survey, participants agree to abide by these terms and conditions. Modak reserves the right to take appropriate action against any participant who violates these terms.
